INFORMATION
Our office is
open Monday through Friday from 8:30 a.m. to 5 p.m. Appointments can
be made by calling the number on the contact page. We can be reached
after hours for emergencies.
Our primary office is located on the campus of John C. Lincoln
Hospital Deer Valley in the John C. Lincoln Deer Valley Medical
Office II at:

A physician is on call 24 hours a day, seven days per week. The
on-call physician may be reached after hours through our answering
service. We ask that routine calls such as information regarding
appointments or prescription refills be made during regular office
hours unless it is urgent in nature. |
